This program provides training
in essential home and self-care skills to support individuals
living in the community. A mobile team travels to the consumers
place of residence, whether he/she lives in a supervisory care
home, residential treatment program, supported/sponsored housing
program, with family or independently. Staff teaches and assists
consumers to gain skills in areas such as:
Nutrition,
meal planning and preparation.
Budgeting and money management.
Clothing maintenance and repair.
Home maintenance and upkeep.
Personal hygiene and grooming.
Coping and social skills.
Staff is available to provide
outreach, and to coordinate with other service providers. An
essential aspect of the program is the collaboration with and
use of other community resources to assist consumers to establish
and maintain these essential supports, independently.
Services are available and may include a combination of individual
and group activities.
