RESIDENTIAL SERVICES
Residential Treatment (transitional service, housing included with treatment), which includes:
FlexCare Plus 24 Designed to provide long-term support to persons with moderate to severe cognitive or functional impairments, this program also functions as a bridge between the behavioral health and medical long-term care systems. Clients receive assistance with activities of daily living in addition to treatment to improve their cognitive, social and community skills. Services are not time-limited and may be provided for as long as there is an indicated need. Services can provide a highly structured treatment setting with 24-hour supervision and therapeutic activities designed to meet the needs of persons who are not in need of inpatient service but require intensive treatment and support in order to gain community living skills. The intended outcome of service is a swift transition from 24-hour treatment to a lower level of community service or independent living.
FlexCare Plus 16 A bridge between an inpatient setting or 24-hour treatment and supported or independent living, these services are highly transitional and focus on developing persons' independent living skills while establishing connections for community integration and ongoing support.
FlexCare Completely integrated in community settings, this is the last level of transitional residential treatment on the road to independent living. In-home and community-based treatment and recovery services help persons gain skills necessary to support independence.
Community Living Support Services
Community Living Program Triple R staff provide intensive in-home and community-based treatment, recovery and support services to persons up to 24-hours a day. These services are not considered a part of the adult transitional residential treatment system and, since the housing is separate from the service, when services are no longer needed the service moves, not the recipient.
Transitional Living Services (intensive, short-term inpatient/jail step down to treatment )
Transitional Living Program Concentrating on quick access to community resources for housing, income and treatment, this program provides short-term support and assistance to persons on their way from inpatient settings to community-based housing or residential programs. Within 30 days, clients and staff work together to identify the next step and marshal the resources necessary to attain it.
Outpatient Services
In-Home and Peer Support Services are provided in a variety of settings, including clients' homes, places of work or other community locations. Peer support services target those isolated and without support systems, who need assistance to get connected to their neighborhood and community.
