The TRHM Program was established
for the purpose of providing a community based independent living
environment for Seriously Mentally Ill Consumers who have been
or are about to be discharged from the Arizona State Hospital
(ASH), or were relocated from Supervisory Care Exit homes. The
program has two basic models for this type of independent living.
Both models use community based
supportive service providers (SSP) to assist the consumers in
their daily living. Under both models, a SSP is co-located on-site
with the consumers.
Model One utilizes apartment settings throughout
the community. Generally, eight apartments for consumers and
one apartment for the on-site SSP are leased by TRHM at an apartment
complex. The consumer lives independently in his or her own apartment
with support from the on-site provider.
Model Two utilized single-family houses located
through the community. Houses from 4-6 bedrooms are leased by
TRHM and then "shared" via a roommate situation in
the house. On-site SSP's while "housed" at the house,
do not have a separate office in the house. This model is a roommate
living arrangement, NOT a group home.
Under the housing program, both
models are considered a "sponsor based" unit. The definition
of a sponsor based unit means that the unit is leased by TRHM
and then subleased to the consumer using an Occupancy Agreement.
Currently the TRHM Program houses over eighty individuals throughout
Maricopa County. All referrals come from ValueOptions Inc, the
Regional Behavioral Health Authority (RBHA) for the County.
